Transportation Forms
Bus Registration Form - WBLAHS - South Campus:
White Bear Lake Area High School - South Campus students entering grades 11-12 in September, who are eligible for transportation, MUST REGISTER for bus service. We can optimize school bus route planning if we know which students will actually ride. Students in grades 11-12 will NOT be listed for service unless we receive this registration form. If your student(s) will require transportation for the coming year, please complete this form and return it to our office. If you waive transportation services at this time, and your situation changes, you may re-establish busing at any time by contacting us. There may be a delay of 2-3 business days before transportation is available. Please contact Transportation and your child's school if any of this information changes during the school year. Postcards with bus information will be mailed in late August. If you have questions, please call Transportation at 651-407-7538.

Bus Stop Change Request:
If, after reading the information on Bus Stop Procedures and Criteria, you would like to request a bus stop change, please fill out the Bus Stop Change Request Form below, and return it to the District Transportation Office. Please include any information about unique of unusual circumstances regarding your child's bus stop. We will review the stop change request and contact you as soon as possible. Please note bus drivers are not allowed to make any changes to a bus stop without prior approval from the Transportation Office. No changes will be made during the first three weeks of school except to accommodate new students moving into the district or those making daycare changes.

Day Care Provider:
If your child needs transportation to or from any location other than your home, you must fill out a day-care form that is available at all of our schools, or download the form below. Return the completed form to the address listed on the form. We cannot transport your child anywhere other than his or her legal residence without the completed day-care form that gives us your written permission to do so.

Kindergarten Transportation Information:
We've created a brochure for new kindergarten students and their parents outlining general information about district transportation services and general information that may be of interest to all families.

Request to Decline Transportation Services:
If you qualify for district transportation services but do not plan to use them (e.g. you will be transporting your child yourself), the transportation department needs a signed form stating that your student will not be riding the bus to and from school. Please complete the form below and return it to the Transportation Office.

Special Services Transportation:
If your child has special needs or requires special transportation services, please complete the emergency health information card below and return it to the Transportation Office.

Non-Public Schools Transportation Request Form:
Non-Public students, who reside within the White Bear Lake School District, and are eligible for transportation, MUST REGISTER for bus service. We are able to optimize school bus routes when we know which students will actually ride. Students WILL NOT be scheduled for transportation service unless this form is completed and returned to our office. If your student(s) will require transportation for the coming year, please complete this form and return it to our office prior to the end of the first week of August. If you waive transportation services at this time, and your situation changes, you may establish busing at any time by contacting us. There may be a delay of 2-3 business days before transportation is available. Please contact Transportation and your child's school if any of this information changes during the school year.
